Prior to your appointment, we will communicate with you and ask you to complete some screening questions. Please go to our COVID-19 Patient Screen Form
to answer the questions. You will be asked to update those same screening questions again when you are in the office.
Please be assured that every effort will be made to screen patients when scheduling appointments. As part of our efforts to minimize everyone’s potential exposure to COVID-19, we will reschedule patients who show any concerning symptoms. Also, prior to your visit, please read our Patient Visit Instructions
for information detailing our ongoing efforts to make our office as safe as possible.
On your visit, you will see some changes to our office. We have installed physical barriers (e.g., plexiglass windows and doors) at reception areas to limit close contact between reception and patients. We have removed items from our waiting area that are difficult to disinfect like magazines. We have equipped our office with HEPA filtration, and in some of our operatories, we have installed wall fans to circulate air and reduce the risk of transmission. We will be requesting that all patients wear masks in our office and one will be provided upon your entry.
